The aim is to provide people with continuous care to improve their health and well-being and to make healthcare more affordable. If your condition is one of the PMB conditions, you can motivate for extra funds. Similarly, once your medications have been approved by your medical scheme, you can often motivate for them to go onto chronic medications to prevent the depletion of your day to day funds.
For your convenience we have attached PMB and chronic medication application forms from certain medical aids. Other medical aids require a clinical letter of motivation from the treating doctor with a treatment plan to request for extra funds. Of importance is that medical aids need to be notified when any changes are made to chronic scripts and PMB benefits need to be reapplied for annually.
